Being aware of my criminal liability, I declare that I am using the services provided by the Bank in my own name, for my own benefit and interest. I undertake the obligation to inform the Bank in writing if I initiate any transaction on behalf of, for the benefit of, or in the interest of a third party, contrary to what is stated in this declaration.
