Trusted Contact Person Consent Form
This form is available for customers of Revolut Bank UAB - Irish Branch to nominate a “Trusted Contact Person”.
To put in place a Trusted Contact Person, this form must be printed or downloaded, signed by the customer and their Trusted Contact Person, and returned to Revolut. For more details, contact our customer support team via the in-app chat.
Section 1: Customer Consent

I understand that Revolut Bank UAB - Irish Branch (“the Bank”) may contact my Trusted Contact Person in any of the following circumstances where:
The Bank needs to confirm:
- my current contact information
- my health status
- the identity of any appointed legal guardian, executor or trustee
The Bank has concerns about possible financial abuse in relation to me or my accounts with the Bank
The Bank experiences difficulties communicating with me
I understand that this may involve the Bank discussing confidential information in relation to such matters with my Trusted Contact Person.
I understand that this consent does not give my Trusted Contact Person the authority to act on my behalf, nor does it make them my legal representative.

Section 2: Trusted Contact Person Consent:

- Retain my name and contact information shared above for the purposes referred to in the above Customer Consent
- Contact me in the circumstances referred to in Section 1
- Discuss the matters referred to in Section 1 with me

- I do not have any authority to act on behalf of the Customer solely by being recorded or contacted as a Trusted Contact Person
- I am not the legal representative of the Customer

This consent remains valid unless and until it is withdrawn in writing by either party or superseded by the appointment of a legal representative.
The trusted contact person has no authority to deal with the affairs of a personal consumer in respect of the account with Revolut, and is not a legal representative for the purposes of Central Bank (Supervision & Enforcement) ACT 2013 (Section 48) (Consumer protection) Regulations 2025, solely on account of having been recorded or contacted by the Revolut as a trusted contact person. Revolut Bank UAB is authorised and regulated by the Bank of Lithuania in the Republic of Lithuania and by the European Central Bank and is regulated by the Central Bank of Ireland for consumer protection rules.
Data Protection – Use of Information
- The information provided by the Customer and/or the Trusted Contact Person on this form will be used for the purpose of contacting you and/or the Trusted Contact Person as instructed above. The information will be held and used solely for the purpose of this instruction. This information shall not be retained for any other purposes.
